With the CeltiCare Preferred comprehensive health plan, policyholders receive:
- Flexible health insurance for self employed and quality individual major medical health insurance coverage for individuals ages six months to 64½ years and their families
- Choice of 80/20 or 100% coinsurance
- Deductible choices ranging from $500 to $5,000
- First-dollar preventive care coverage
- Healthy Lifestyle cash incentive
- Rx coverage - $20 copay for generic drugs with no deductible
- Additional Options including the Prescription Drug Option, Supplemental Accident Option, and term life insurance (not available in all states)
Applicants can choose from three benefit-rich individual major medical health insurance plans:
CeltiCare Preferred Select PPO (Doctor and Hospital Network)
Policyholders receive high quality care for the lowest premium by accessing participating network providers. This new physician and hospital PPO plan offers savings on every visit to any network provider with a low $15 copay.
CeltiCare Preferred "Any Doc" PPO (Hospital Network)
With the "Any Doc" PPO plan, policyholders don’t have to change doctors to realize the advantage of a low $35 office visit copay. Offering the flexibility to choose any physicians while saving money with preferred rates at prominent network hospitals, this plan is ideal for people who want to visit the doctors of their choice with no network restrictions.
CeltiCare Preferred Managed Indemnity (No Network)
Provides traditional, comprehensive individual major medical health insurance for individuals and families who want the flexibility to use the doctors and hospitals of their choice.
